Riverview Adult Day Health Center Alerts Community to New Medicare Outreach on Dementia Care Support

Riverview Adult Day Health Center is notifying local families that the Centers for Medicare & Medicaid Services (CMS) has begun mailing an important informational letter to millions of Medicare beneficiaries diagnosed with Alzheimer's disease or other forms of dementia.

 

As part of a nationwide effort, CMS is sending more than four million Beneficiary Letters to introduce the GUIDE Model (Guiding an Improved Dementia Experience)--a new initiative designed to expand care, resources, and support for individuals living with dementia and the caregivers who support them.

 

The letter contains general information about the GUIDE Model and outlines new benefits now available to eligible Medicare recipients. Beneficiaries in the Greater Fall River region may receive this communication over the coming days or weeks.

 

Riverview Adult Day Health Center, through its partnership with **Harmonic Health--one of the largest participating organizations in the GUIDE Model--**will be offering access to comprehensive, no-cost dementia support services, including:

  • Up to $2500 per year in respite care
  • Caregiver coaching, education, and assistance
  • Personalized dementia care planning
  • Help understanding program benefits and next steps
